Food Service Information

St. Patrick's offers food service tickets for students. To order, include a list of what you are purchasing and send the check to school.

Reduced prices are available for qualifying families:
Breakfast

*Students receive milk with breakfast and lunch. A separate milk ticket would be used if the student brings his/her own lunch from home or wants an extra milk or a snack milk during the day. Kindergarten students only need a milk ticket if they are bringing cold lunch to school.

Attached below are required forms including the USDA required diet modification form, free and reduced meal income requirements.

may be made available in languages other than English.

To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at: http://www.ascr.usda.gov/complaint_filing_cust.html, and at any
USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992.
